You Referred Them.
We Make Sure You Were Right.
When you put a patient's name on a referral, your credibility goes with it. We move fast, document everything, and keep you in the loop — because a good referral relationship is built one well-executed placement at a time.
Why Partners Keep Referring Back
We know you have choices. Here's what we do differently — and why it matters to the patients you place and the outcomes you're accountable for.
48-Hour Care Initiation
From the moment we receive the referral, we're working the authorization, matching the aide, and scheduling the first visit. We don't fill a slot — we stand up a care plan. The first visit includes a fall-risk walkthrough and medication review, so day one means something.
Typical intake-to-schedule: 24 hrsRN Oversight on Every Client
A registered nurse reviews every care plan, maintains physician communication, and watches for clinical changes before they escalate. This isn't a premium tier — it's standard at AboveAll. When something shifts, we call the physician and we call you. Not after the fact.
100% RN-supervised care plansFull EVV Documentation
Every visit is captured through Georgia's Electronic Visit Verification system — timestamp, location, duration, tasks completed. No gray area, no guesswork. If DCH or a case manager ever needs to audit a client's record, we have it. You referred someone into a compliant operation.
100% EVV-verified visitsConsistent Caregiver Matching
Once we match a client to an aide, we protect that relationship. Rotating caregivers aren't just disruptive — they're a clinical liability. Our clients and their families know who's coming, which means better adherence, faster trust, and fewer calls back to you asking what happened.
Consistent aide assignment policyFamily Integration & Education
We train family members on what to monitor between visits, how to support medication adherence, and when to escalate. A family that knows what's happening is an extension of the care team — it reduces unnecessary calls, improves outcomes, and protects the client when we're not there.
Family caregiver training includedWe Actually Pick Up the Phone
If you've placed patients with agencies that go quiet after intake, you know how much time you spend chasing updates. We keep you informed on condition changes, flag anything unusual, and return calls. You stay informed without having to hunt us down.
Same-day response to partner callsFrom Referral to Care — Step by Step
We've built the intake process around what discharge planners and case managers actually need: speed, transparency, and no back-and-forth.
Send the Referral
Call us or submit the form on this page. We need: client name, county, Medicaid ID or pay source, and what care is needed. That's it to get started.
Takes under 3 minutesWe Handle Authorization
Our team contacts Medicaid, processes the prior auth, sets up EVV enrollment, and identifies the right aide match — all without pulling you back in.
Typically 24–48 hoursFirst Visit: Full Assessment
Day one isn't just a check-in. We complete a fall-risk walkthrough, an RN medication review, and care-plan alignment with your discharge summary.
Within 48 hrs of referralYou Stay in the Loop
We proactively flag condition changes, document everything through EVV, and reach out to you — not the other way around — when something shifts.
Ongoing partner communicationWhat Every AboveAll Aide Brings
When you refer a patient to us, you're referring them to a vetted, trained, compliant team — not a staffing pool. Here's what every aide in our network carries before their first visit.

What We Accept
We work with Georgia's primary home care Medicaid waivers, as well as private pay and long-term care insurance. If your patient's funding source isn't listed, call us — we'll tell you straight.
EDWP: CCSP
Georgia's primary home and community-based waiver for older adults and adults with physical disabilities who would otherwise require nursing facility level of care.
EDWP: SOURCE
Managed care waiver for individuals who meet nursing facility criteria. Coordinates medical and long-term services and supports through a care management entity.
ICWP
Serves adults with physical disabilities between ages 18–59 who need nursing facility level of care and choose to live in the community.
Private Pay
We accept private pay and can coordinate directly with long-term care insurance carriers. Call us to discuss rates and coverage verification.
What Referral Partners Ask Us Most
Straight answers to the questions discharge planners and case managers ask before — and after — their first referral.
-
How quickly can you get a client started after I send the referral? For hospital discharges and urgent transitions, we target care within 48 hours of receiving the referral — often sooner. We begin authorization and matching immediately, not after a multi-day intake queue. If the situation is critical, call us directly. We move differently for urgent placements. -
What if I'm not sure whether my patient qualifies for Medicaid home care? Call us before the patient is discharged. We can help you determine which Medicaid program — EDWP, ICWP, or GAPP — fits the patient's age, diagnosis, and functional level, and whether they meet nursing facility level of care criteria. We've worked through a lot of these situations and can give you a fast read on feasibility. -
How do I know if you serve the patient's area? We currently cover 18 Georgia counties. See the full list. If your patient is in a county not on this list, call us — we're expanding and may already be able to serve the area. We won't leave you guessing. -
How will I know if the client's condition changes after placement? We flag condition changes to the referring partner proactively — you shouldn't have to call us to find out what's happening. If we observe a change during a visit that warrants a care plan adjustment, or if we contact the physician, we loop you in. We also document every visit through EVV, so there's always a timestamped record you can reference. -
What happens on the very first visit? Day one is a full start — not just showing up. We complete a home safety and fall-risk walkthrough, our RN reviews the medication list against discharge instructions to flag any gaps or conflicts, the aide introduces themselves to the client and family, and gets aligned with the care plan. We believe the first visit sets the tone for every visit after it, and we treat it that way. -
Can I send referrals for clients on private pay, not just Medicaid? Yes. We accept private pay and long-term care insurance in addition to Georgia Medicaid waivers. The same standards apply regardless of pay source — same credentialing, same RN oversight, same EVV documentation. Call us to discuss rates and how we handle insurance coordination.
