The Georgia Pediatric Program (GAPP) is a Georgia Medicaid program that provides in-home skilled nursing and personal care support for medically fragile children and young adults, so they can live at home with their families instead of in a hospital or facility.
Who GAPP is for
- Children and young adults under 21
- Who are Medicaid-eligible
- Who have complex medical needs that require skilled nursing care at home, for example a tracheostomy, ventilator, G-tube feeding, or frequent seizures
What GAPP can cover
- In-home skilled nursing by an RN or LPN, for a number of hours approved in the child's plan
- Personal care support for daily needs
- Coordination with the child's doctors and specialists
How to get started
- Talk with your child's physician. GAPP requires a physician's order and documentation of medical need.
- Choose a home care agency that provides GAPP services. The agency typically helps submit the request.
- The request is reviewed and, if approved, nursing hours are authorized and periodically re-reviewed.
Tips for families
- Keep a binder with medications, equipment suppliers, and emergency plans.
- Ask about nurse consistency. Familiar nurses matter for your child's comfort and safety.
- Plan ahead for school transitions and the move to adult services at 21.
